Care Provider Information Space
The care provider page is an address book of all your professional care providers. It's not intended for personal use, because that could compromise your anonymity from the journal. A care provider can have multiple Patients with multiple pat. ID's. Make sure to fill in the Pat. ID because this is how your book will find you if you lose it.

The medications page is were you will list all your prescriptions and supplements that you use on a regular basis. With the help of your licensed care providers and/or pharmacist schedule your medications for time and compatibility using the color coded stickers, applying them to the proper position of each prescription. This is also the page that you will attach your memos making sure to reference the proper patient.

The calendar is to be used for scheduling appointments for all patients in the journal, this will help you avoid conflicts in scheduling and will let you schedule appointments to be more economical as well.
Medical History Keeper
The patient history is a reference section that you will list your history starting with your past to present, you will also list any and all allergies here. This is a good reference when starting up with a new doctor, it gives you dates and eliminates accidental omissions when filling out paper work. It is a great reference in an emergency whether it is needed at home or at the hospital, it provides all emergency care providers accurate and up to date information that may expedite your care.

Daily Vital Sign Tracking System
The daily log is were you will keep a record of your daily vital signs and make a comment if you experience any changes in how you feel. With this daily log you will be able to pin point any hospital stays by the omission of entries, because while in the hospital you will not be making any entries.

Memo Pad
If you want to write memos to your doctors while at home we suggest that you have a self adhering pad available to use. The journal has a limited supply of the two part memos which will give you a record of the notations you have passed on. The best place to post your memos is on your medications page of the journal, because medications get verified at each doctors appointment.
